WHAT TO BRING
Please bring the following to your initial consultation:
1. Medicare Card
2. A current list of medications
3. Relevant radiology and other test results
4. For chronic or complex problems, bring a short written medical history. Include a list of healthcare professionals attended for your current problem, treatments received and your responses to those treatments
REFERRALS
A referral is not required for an appointment at Equilibrium Medicine.
Please note
It is recommended you wear suitable loose clothing (i.e. shorts and singlet) that adequately allows for examination. However, these can be provided if required.
Bringing of a support person with you to your initial consultation is strongly encouraged.
It is not advisable to bring small children to your consultation. Conducting a thorough assessment and formulating a management plan requires your full attention.
Please arrive 15 minutes prior to the appointment time of your initial consultation to complete the necessary paperwork.

A proportion of Dr. Choong's fees qualify for Medicare rebate, subject to the standard Medicare conditions. Equilibrium Medicine is a private billing clinic.
Private Health Insurance rebates do not apply.
REFERRALS
A referral from your GP is not required for an appointment at Equilibrium Medicine.
CANCELLATIONS
Please call as soon as possible should you wish to cancel. Cancellations within 24 hours of the appointment may attract a cancellation fee.
